Upon submitting a new patient request and choosing a membership your information will be stored, but no charge will be submitted until the first visit.

Tier 1 - Core Care - Month-By-Month Membership Sign-Up Request - $200 billed monthly
Tier 2 - Core Care Plus - Annual Membership Sign-Up Request - $3850 billed annually
Tier 2 - Core Care Plus - Month-By-Month Membership Sign-Up Request - $350 billed monthly
Crome Medicine is NOT insurance. Crome Medicine provides a direct physician relationship built around time, access, continuity, and a structured approach to long-term health. Health insurance is recommended for all patients. Crome Medicine may retain your health insurance information to facilitate referrals, order tests and coordinate with other health care providers.
