What Membership Includes
Crome Medicine is a direct primary care (DPC) physician-owned practice. It is designed to accept direct payment from each member to provide primary care as described below.
Membership Options
Tier 1 - Core Care: $200/month
Tier 2 - Core Care Plus: $350/month
Core Care for All Members (Tier 1 and Tier 2)
Annual comprehensive evaluation
6-month follow-up visit
Quarterly check-ins (in person or virtual)
Routine, problem-focused, as-needed acute, telemedicine, and phone visits
Secure messaging through EHR portal
Medication management and coordination of care
EKG, body composition analysis, selected in-office procedures, and rapid testing
Core Care Plus (Tier 2)
Planned monthly physician check-ins (in person or virtual)
early and late priority scheduling
coordination with a personal trainer
direct text communication
advanced care coordination
advisory hospital visits

annual and 6-month laboratory evaluations

Important Consideration
Membership covers the primary care services described by Crome Medicine. It does not replace health insurance and does not cover hospitalizations, emergency care, specialists, imaging, medications, or external services not included in the membership. Maintaining major medical insurance is strongly recommended.
The 6 Domain Framework
The six domains are the proprietary operating structure of Crome Medicine. They help translate health from a general aspiration into a practical system: define the behaviors, understand the person, improve and stabilize cognition and mood, build the body, measure the results, and support it all through nutrition.
I. Daily Discipline - Physiologic order as the foundation of preventive medicine.
Optimize sleep, circadian rhythm, exercise, nutrition, alcohol regulation, cognitive load, and self-monitoring.
The aim is rhythm, not perfection. Stable daily signals reduce physiologic strain and make progress repeatable.
II. Personal Growth & Life Direction - Directed identity as a physiologic regulator.
Connects discipline and goals to purpose, responsibility, relationships, work, and aging.
When a person knows what he is building, health behaviors become less dependent on motivation and more connected to identity.
III. Mental Health & Cognitive Performance - Cognitive performance as the aim; emotional stability as the foundation.
Protects the brain systems that regulate attention, motivation, judgment, emotional control, and disciplined action.
The focus is not only illness treatment, but preservation of clarity, steadiness, and long-term cognitive durability.
IV. Physical Health & Performance - Physiologic capacity as the aim; durability as the standard.
Builds and preserves muscle, strength, aerobic capacity, mobility, metabolic function, recovery, and independence.
The goal is a body capable of doing what life requires - now and decades from now.
V. Measurement & Physiologic Feedback - Objective data as the foundation of precision health.
Makes health visible through the right data, interpreted over time: body composition, labs, cardiovascular risk, VO2 max, sleep, HRV, recovery, exercise, nutrition, sleep adherence, and preventive screening.

VI. Nutrition as Fuel for Life - Fueling the body and shaping the experience of living.
Creates nutritional structure that supports body composition, metabolic health, training, energy, relationships, and real life.
The goal is not dietary ideology; it is awareness, consistency, flexibility, and measurable results.
The domains are not six separate programs. They are one integrated clinical model. When one area weakens, the others become harder to sustain. When the system strengthens, behavior becomes more reliable, physiology becomes more stable, and health becomes more actionable.
How Care is Delivered
Crome Medicine uses a structured annual schedule while preserving flexibility for acute concerns, unexpected problems, and individual needs. The model is intentionally longitudinal: the physician and member review where things stand, identify what matters most, execute a practical plan, and recalibrate based on results
The Member Journey
1. Establish Baseline
Clarify history, goals, medications, risk, current behaviors, sleep, nutrition, exercise, stress, cognition, relationships, labs, and function.
2. Build the plan
Identify the highest-yield priorities across the six domains. Translate goals into a small number of behaviors, visits, testing decisions, and follow-up intervals.
3. Execute and monitor
Use planned visits, communication, acute access, and selected data to keep the plan visible. Prevent drift before it becomes relapse or disease progression.
4. Recalibrate
Review trends, interpret results, adjust medications or lifestyle prescriptions, and update the plan as goals, physiology, life demands, and risk change.
